Do I need to fill out this form before purchasing classes? Yes. If you have not filled out this form and made a purchase, you can also come into our office and fill out a physical copy.
Is this related to the Sign In/Account Registration page? No. Registering for an account on our website is so that you can easily make a future online purchase without having to re-enter your information. The Student Registration Form is for our record keeping purposes only.
